a)	Indicate how many years you have been receiving instruction in your chosen major musical discipline and what type of instruction (i.e. private lessons, school music program, etc.)
b)	Indicate how many years you have participated in the Midland Y’s Men’s Music Festival, and in which disciplines;
c)	Indicate and give pertinent details of any other music festivals or music competitions/workshops in which you have participated;
d)	Indicate your participation in school music classes and extra-curricular activities at the elementary, secondary and if applicable, post-secondary levels;
e)	Indicate any involvement you have had in other musical organizations or activities within the community;
f)	Indicate at which post-secondary institution you will be studying music in the fall;
g)	Give a basic outline of the music curriculum that you will be following at the post-secondary institution you will be attending;
h)	Indicate any long-term goals or career plans you may have in the field of music at this time;
i)	Provide any other information about yourself that you feel is relevant to this application.

Midland Y’s Men’s Music Festival

SUTHERLAND AWARD OF DISTINCTION BURSARY

The aim of this bursary is to provide significant financial support to a Music Festival participant who will be studying music as a full-time student at a post-secondary institution. Since its inception in 1999, this major award has been made possible through the generosity of Mr. Bill Sutherland of Sutherland Financial Services of Midland. It will be administered by the Midland Y’s Men’s Music Festival Committee.

ELIGIBILITY

The applicant must be planning to attend a post-secondary institution (university, college, or recognised conservatory of music) as a full-time student in the calendar year of the current Music Festival. The bursary will be paid on presentation of proof of acceptance to the post-secondary institution.

The committee will give favourable consideration to those who perform in five or more classes in one or more disciplines with the highest average adjudicated mark.

TERMS

The successful applicant will be selected by the Midland Y’s Men’s Music Festival Committee.
The successful applicant will be announced at the Festival of the Stars.
The Bursary must be used within two years of being awarded.

EVALUATION OF APPLICANTS

Midland Y’s Men’s Music Festival participants who are interested in applying must complete the application form in detail and mail it to the following address by 1 April, 2012
